  • A blood or marrow report raising the possibility of leukemia
  • A confirmed diagnosis needing a treatment plan or a second opinion
  • Follow-up after treatment completed elsewhere

  • Bone marrow, flow cytometry, cytogenetic and molecular reports
  • All blood counts from the start of the illness
  • Discharge summaries and chemotherapy records
